Listen Up for the Rules!

1.
The 2011–2012 Refer a Colleague program runs May 1, 2011, through April 30, 2012, for all AGD members (excluding dental students, who are eligible to participate in the Refer a Classmate program). Applications and dues payments for new members must be received by AGD Headquarters and processed on or before April 30, 2012, to qualify.* Those processed after April 30, 2012, will be applied toward the 2012–2013 Refer a Colleague program.

*Please keep in mind that mailed applications and payments may take as many as three weeks to fully process. For fastest processing, the AGD recommends faxing applications and credit card payments to 312.335.3443. Should you choose to mail applications and payments to AGD Headquarters, we strongly encourage you to postmark all materials no later than April 6, 2012, to guarantee credit in the 2011–2012 Refer a Colleague program.
2.
A new member is officially credited to you once his or her application and dues payment have been fully processed. To receive credit for a new member, your name and city/state must appear in one of the following: the “Referral Information” box on their paper application; Step 2 of the online application; on the online referral form; in an email sent to refer@agd.org; or via verbal communication to an AGD Membership Services Representative at the time that the new member joins over the phone.
3. To be considered for prizes, rewards, recognition, and a Top Recruiter title in the Refer a Colleague program, each new member you recruit must be an active general dentist, affiliate, or associate member.

An active general dentist member is defined as one of the following:
right arrow A dentist who graduated from an accredited dental school in the United States or Canada;
right arrow A dentist who has successfully completed an accredited general practice residency (GPR) or advanced education in general dentistry (AEGD) program in the United States or Canada; or
right arrow A dentist who holds a license to practice dentistry in any state or territory in the United States or province of Canada, but is currently practicing in a country outside the United States or Canada.

An affiliate member is defined as a person not eligible for any other type of membership in the AGD but who supports the aims and objectives of the organization.

An associate member is defined as a dentist who graduated from an accredited dental school and holds a license to practice dentistry in any state or territory in the United States or Canada but is practicing as a specialist rather than a general dentist.
4.
All members currently serving on the AGD House of Delegates or Board of Trustees, or as elected or appointed officers of the AGD, are included in the Volunteer Leader recruiter category. All other members are included in the Grassroots Member recruiter category.
5.
You will be awarded one complimentary Self-Instruction exercise for each new active general dentist, affiliate, or associate member you recruit through the 2011–2012 Refer a Colleague program, up to a maximum of 10. Each new member will be awarded three complimentary Self-Instruction exercises.

To qualify, new member applications and dues payments must be received and processed between May 1, 2011, and April 30, 2012. Exercises expire one year from the date on which they are processed.
6. All prizes and rewards, with the exception of Self-Instruction exercises, may be used by the winner or given as a gift to another AGD member dentist.
7. Program rules and prizes are subject to change by the AGD. Any changes made to the program will be posted to this page.

Refer a Classmate Program

Since they do not apply to the Refer a Colleague program, any new dental student members you recruit to the AGD will instead be credited to your constituent toward the Top Constituent Recruiter of Students title and prize in the Refer a Classmate program.

Current dental student members who recruit their classmates to the AGD will receive individual credit toward their own prizes, rewards, recognition, and the Top Student Recruiter title in the Refer a Classmate program.

A dental student member is defined as a pre-doctoral student of an accredited dental school in the United States or Canada.
