Fellowship Examination Application
Apply to take the exam at the annual meeting, LaserGrade centers (computerized exam), or a Defense Activity for Non-Traditional Education Support (DANTES) center for military personnel outside the U.S. and Canada.
Take the Fellowship Exam On Computer
The computerized version of the Fellowship Exam is offered year-round at LaserGrade testing centers throughout the U.S. If you wish to take the computerized exam, complete the application at least two weeks prior to the date you select for testing. During the months of October, November, and December, registration fills up quickly so be sure to call to schedule your appointment at least three weeks in advance of the date you plan to take the exam.
The Academy of General Dentistry (AGD) will process your payment, alert the testing center that you are eligible to take the exam, and contact you with information on how to schedule a testing appointment.
Computerized exams can be taken at LaserGrade testing centers five days a week (Saturday testing is available at select locations), excluding holidays. No prior computer experience is necessary. NOTE: You cannot make an appointment directly with a testing center until you receive confirmation from the AGD.
Take the Paper and Pencil Version of the Fellowship Exam
The AGD provides members with the opportunity to take the Fellowship Exam on paper and pencil once a year at the AGD Annual Meeting & Exhibits. You can register separately for the exam or with your annual meeting registration. You may also register on-site to take the exam up to and including the morning of the exam.
DANTES Testing Centers for U.S. Military Personnel
Members who are in the military and stationed overseas may register online to take the exam at a DANTES facility, given in conjunction with the annual meeting examination date. A letter must be submitted to the AGD from the DANTES Test Control Officer (TCO) verifying the following information:
- Name of the proctor administering the exam;
- Shipping address;
- Contact telephone number or e-mail address;
- Confirmation of the member name(s); and
- A statement confirming that the TCO will administer and secure the exam until such time when it is returned to the AGD.
Members will not be allowed to sit for the Fellowship Exam if a letter is not submitted from the DANTES TCO. Letters may be mailed, faxed, or e-mailed to the AGD.

Cancellation Policy
Computerized Exam: To avoid a penalty, you must cancel at least 48 hours in advance of your testing date through LaserGrade's registration number at 1.800.211.2754 (M-F 8:00 a.m. – 7:00 p.m. CST; Saturday 8:00 a.m. – 4:00 p.m. CST). Less than 48 hours notice will result in a penalty forfeit of the entire examination fee.
Paper and Pencil Exam: Canceling the paper and pencil version of the Fellowship Exam less than 20 days in advance, or failure to cancel will result in a cancellation fee of $50.
Retaking the Exam
If your Fellowship Exam score is within five points of a passing score, you may retake the next version of the exam at no charge at the AGD Annual Meeting & Exhibits, or for $150 at a LaserGrade testing site. Special Accommodations
The AGD will grant special accommodations for the Fellowship Examination to a member taking the paper and pencil version of the FAGD Examination who:
- Submits a letter, a minimum of 60 days before the examination deadline, requesting special accommodations, and
- Provides documentation verifying the member's condition as well as the specifics of the special accommodations from a qualified professional (physician, psychologist, counselor) currently treating the candidate.
The AGD reserves the right to authorize the use of auxiliary aids or modifications in such a way as to maintain the integrity and security of the examination process.
