You have one last step to LLSR!

To receive your Lifelong Learning & Service Recognition (LLSR), you must submit the following to the AGD:

• An application
• Written verification of your volunteer activities
• A current AGD transcript, and
• The required fee.

Download the LLSR application , or request the application via e-mail at awardapp@agd.org. Applications are reviewed by the Council on Dental Education on a bi-monthly basis.

*The LLSR is not a credential and in no way may be represented to the public as such. A Master may receive the LLSR multiple times, in a sequential manner, as long as all requirements are met. Once a Master is first recognized by this achievement, subsequent recognitions may only include those credits and points earned since the date of the previous LLSR recognition.
